He was dying from the plague when we last saw him. Then he died from the plague.

He was all sickly and we saw him vomit while in the tomb. He had the plague.

He had a huge sore of some sort (plague) on his face and vomits blood up while leaning over Anna's corpse. So, yeah, I'd say "plague."

Although, the film also has Orlock state that he will be murdering Ellen's loved ones, and while she had a strained relationship with Friedrich at the end, I think this was still part of the Count's haunting of Ellen. Orlock also brings the rats and plauge. His shadow reaches out across the city and brings death and disease. Directly or indirectly, it's Orlock who infects the city - including Friedrich - to die of plague.

Actually, I thought the sunlight killed him which is how he died in the original.

Ellen having sex with him distracted him from the fact that the sun was rising.
