And what, based on a few words, Miggs would just develop a conscience like that and agree to it?

shareThe only way it's physically possible for a human being to swallow their own tongue is to slice it out with something sharp, which prisoners are not allowed, and swallow it like a piece of raw meat which wouldn't choke you. A person can't swallow their own tongue as is, it's quite firmly attached to the jaw and throat.

Miggs was obviously extremely mentally disturbed, and Lecter would have known exactly what his psychological trigger points were. Couple that with possible epilepsy and Lecter most likely sent him into an uncontrollable fit.
One can only imagine what junior high Lecter was like in the playground.
The same way Nurse Ratched caused Billy Bibbits suicide. They were both masters of psychological manipulation and knew how to trigger their victims
