I'm not sure I'm well versed enough in movies to have a top 20 but I like these a lot:

The Shining (1980)- in fact this is probably my second favorite movie of all time, any genre
Misery (1990)
The Omen (1976)
The Exorcist (1973)
Not sure if Rocky Horror Picture Show counts but if so then it's another of my favorites.

I remember seeing part of Phantasm (1979) and being intrigued. I should watch the whole thing sometime.
