So this guy is counting people as they come back on the boat. 2 guys leave again and when they return he counts them again, thus thinking he had 20 on the boat.

The problem: The girl assisting people on and off the boat had to have known the procedure he was using, so she neglects to inform him? And why would you not take a final count BEFORE you sail off. Totally asinine.

Since this is based on something that really happened you can read up on their incompetence. But happen it did.

I did. There was nothing specific about how it happened. They captain was brought up on charges, found not guilty, and the company later went out of business.

It was incompetence. But so many people are incompetent at their jobs that it's not unusual. However when your job can mean life or death for your clients, you should go the extra mile and be SURE you get things right.

If I had a tour boat I'd be terrified of leaving a diver behind. I'd definitely put in place several safeguards. For instance, I'd have a printed list of everyone getting onboard. I'd have everyone sign his name under the printed one. And when they boarded after diving, I'd have them check off their names. I'd do another head count and then I'd say the list of names and have everyone respond with "Here" or "Present".

I've waited tables and felt terrible if I forgot someone's drink order. I'd be devastated if I forgot someone at sea!!
