I felt this couple got tacked on to give Michael Jr. a place to go after his dad's death.
At that time period there would have been lots of random farms spaced all throughout the country side, and most any farmer could have treated a gunshot pretty much on par with a doctor. Penicillin wasn't available so all you could really do beyond removing the bullet was hope the person's body could recover from any infection.
