I think most people don't understand planetary physics (which is understandable). No one can ever really float away perse. They will be stuck in an orbit around Earth for a very long time and eventually reenter because 250 miles up, Earth still has a thin atmosphere that slows down objects in orbit. The only time this could have happened (but fortunately didn't) was during the Apollo moon missions because they were leaving Earth's sphere of influence to enter the Earth's, and a couple times they were on a non-free return trajectory, so if there was an accident they could have went into an orbit around the Sun for eons. Anyway moving on...
For one, like everyone else here said, there were a few rare exceptions. Three cosmonauts one of you mentioned is true, their capsule became depressurized and they died before even landing. This was probably the closest there was (confirmed anyway) of humans died "in" space. Also Columbia disintegrated in the upper atmosphere. Another infamous now uncovered mission is of Vladimir Komarov. His ship survived reentry but the parachutes failed to deploy and his Soyuz ship smashed into the ground if I'm not mistaken at speeds of 50m/s. If you have a strong stomach, there are pictures of his charred remains at his funeral. Not super morbid but it's kind of shocking all that's left of him is a 3 foot long pile of unrecognizable human remains.
The space program is in shambles enough, I pray that no more humans are killed in any endeavors in space.
Lets not forget the cases that were never reported... Missions that were never made public! Stuff thats not in history books... Rumor has it that old Soviet space program sent cons into space after the dogs... Before Gagarin... The con would be pardoned if he returned....
They never found the second dose!

I'm sure I once read up about a Soviet mission from the 70s or 80s in which a suited cosmonaut nearly died this way by accidentally letting go of his orbiting spacecraft, and was only saved by his colleague managing to grab onto him.
Also in the 80s, Space Shuttle astronauts used the Manned Maneuvering Unit for a few missions, which was basically an untethered spacesuit with a rock pack on the back. But following the Challenger disaster, it was considered too much of an unnecessary risk and only tethered spacewalks were used from then on. Now that sounds to me that it didn't matter if you were only a few metres away from your spaceship; if your rocket pack broke down, there was no way to get back!
