Or is his football head just a cartoon trait to make him stand out?
Similar to Sid's long nose, Harold's sack looking head, Patty's Large size, Helgas unibrow, and Grandpa Phil's penis shaped head (lol well it's true).

I don't think he's supposed to be deformed. Like you said it's a cartoon trait to make him stand out like the other characters' physical traits.

I think Helgas' unibrow is really suppose to be a unibrow. Today, thick eyebrows are coming back.

It's a weird family trait. Arnold's mother and his cousin Arnie also had that head shape.
