Is it because she's perpetually cheery and optimistic? Because of how up-front she is with Arnold about the fact that she doesn't "Like him, like him"? Because all the other characters (except Helga) really like her?

I dislike her because she's a caricature, and not an entertaining one.
I do enjoy funny satirical characters that are designed to make fun of a given type of person/profession in society - I love Mr. Shelacki and Mayor Bob White on Doug, for instance - but she just serves no purpose other than to be an annoying Little Miss Princess Goody Two Shoes.
I think it's because Helga was against her and we love Helga!

Helga might be a bully, but she wears her emotions on her sleeves, she has no inner malice to hide. If anything, we see why she's a good person deep down. Lila in option two...she's not only messing with Helga despite knowing her secret, but she's messing with Arnold too by purposely blowing hot and cold. And being utterly fake in the negative sense. Thankfully, something clicked in Arnold in the episode "Timberly Loves Arnold" and his crush was through. Guess his subconscious suspicions of who she really was (seen in "Arnold Visits Arnie") dwelled into his conscious mind.
Lila in option one...is just a bland, goody two shoes. There's no flavor, no substance.
None of them sound positive or likable (though option two makes her a lot more interesting). Add in the fact that she's Helga's rival and everyone wants Helga/Arnold...let's say it makes it very difficult not to hate her.
Me, though? I'm neutral.

Maybe people disliked her because she had an eye for Arnold's lint-collecting, snorting cousin. Naw in all seriousness, I agree with the theory of Helga's perspective in seeing Lila as a threat as to the reason people may dislike her I mean Helga has had strong feelings for Arnold since day one and when Lila came along her chances of ever having a relationship with him were pushed even further away. It's like when you apply for a job, sometimes other applicants pose a threat, doesn't make them a bad person.

'Big Sis' is a good example of this. Lila starts off as a sweet, introspective girl (which still shows through later, but only a little) but later she and Olga are both completely insensitive to Helga.
Personally, I never disliked Lila, but I think her character was written worse toward the end. I think the writers themselves must not have been big fans of Mary Sue type characters. While other characters gained depth as the series went on, Lila lost depth. Even her crush on Arnie affects her depth, since there was never a real reason for her to like him.

From what I hear, many people hated Lila because they thought she was a bitch to Arnold bu manipulating and rejecting him. Some who felt she led him on. But I don't agree. Lila was pretty honest about not liking Arnold. Anything else that happened was on him, for win a girl over.
But otherwise, I agree with other answers. They hate her because Arnold likes her. And that's
1. Bad for Arnold fangirls who like him, and due to that, they probably hate Helga too, since Arnold is supposed to end up with her.
2. They hate Lila because Helga does, and they want Arnold to forget L and date H.
