Because different countries are different countries, and the 1960s (when Doctor Who first came to be) were the 1960s.
I asked my mother about Doctor Who once to know whether the Classic series ever aired in my country (neither US nor UK) and she didn't know the show at all. TV simply wasn't as international as it is now before the internet, and nowadays there's simply so much variety that it's impossible for everyone to be familiar with every well-known show. I know the names and general ideas of the most famous and popular old shows of my own country even if they were before my time and I've never watched them and they sound dumb, just because they are part of my pop culture history. Doctor Who is a big part of popular culture in the UK and some other countries, but that doesn't mean it's so everywhere, the same goes for most shows (there are exceptions like Star Trek, Friends or Game of Thrones which pretty much everyone knows even if they've never watched a single episode, but most shows aren't like that.) Like I remember once reading some list of the most watched TV shows ever in the US and there were things like M*A*S*H or Cheers that I had never even heard of before that.
