Who killed Cortez?
Did the demon manifest itself into our reality and kill Cortez, or was it Sean himself who did it while off his meds?

The demon Cortez conjured up killed Sean--and him.
I think he had already successfully conjured the demon when the movie began---you can hear it growling or whatever that creepy sound was, in the early scenes. Also, there is at least one scene in the woods where viewers definitely see something and it isn't Sean or in Cortez's imagination...
More specifically around the 1:05-1:06 mark is when viewers can see what happens.
