I don't mind some spoilers, and I'm just wondering: How does his wife die?

Accident. That frankly, is majorly glossed over. What kind? your guess as good as mine.
Somehow get impression he was driving or somethin' (maybe drunk, maybe just bad luck......who knows). Kinda matters actually.

Again, thoroughly glossed over.
Shouldn't have been. Takes 2 minutes for him to just tell exactly WHAT happened to another so we can see why he was so broken & angry. (at start of movie)

Thanks. That's what I thought. Makes sense, he feels guilt over her death.
