She missed out on a residency.
She did her internship in the emergency dept but always wanted to do her residency in pathology. But the longer she worked in emergency the more she realized she no longer had an interest in pathology and wanted to stay in the ER.
When she graduated she got the residency in pathology, but decided after all not to take it which left her nowhere to go and with no job.
She had to take the job in the coffee shop until she could either find another residency opening or another job somewhere.
But in a recent episode there was that resident who really wasn't cut out for the ER. I wonder if he's going to stay on or if he'll leave. Then there will be an opening for Sarah and she'll have to decide if she wants to take it or stay in psychiatry with Dr Charles.

Thank you! I hope that Sarah eventually gets her act together. She needs to learn as much as she can from Dr. Charles. He's a wise mentor, and he's actually my favorite character on the show.
