How do we know that they didn't just add that in the commercial for dramatic effect? It's like in several advertisements they have Elena's voice saying "Do you think you're the first woman whose tried to save him?" as we see a clip of Leila standing behind Ana and Ana turning around. Making it appear that it was Leila saying it.

They don't make scenes for dramatic effect in trailers, they might cut two different scenes together, or like you said voice over on another actress, there is no Ethan so the gun alerts Grey that Ana is in trouble!

I'm not so sure about that. As protective as Christian is towards Ana he would be busting through the door not just stepping through like he did in the trailer's if he heard a gunshot going off.

Correct, per the book there was no gun fired. I am sure they changed it up a bit to make it more "dramatic"?!?!
Per book, Ana talks to Leila and manages to de-escalate the situation until Christian storms in the door!! No gun was fired in book.
