So he chose... (spoilers)
Sarah over Kate? Is that what was implied? Or was it more open-ended.

That's how I saw it. He was with her at the hospital when she had the baby. Then the tragic outcome, that made him stay with her.
shareBut after he found out that Sarah had died and been brought back his demeanor changed and he left the hospital.
sharehe went outside to make a phonecall, that's not the same as leaving
his reaction to her possible (practically certain) death and resurrection isn't so simple either. keep in mind that he just had to kill a former friend who 'came back wrong' and Dr Secrets implied that something else was possessing him... he, just like we are, has to be considering that she might not be herself now

However had she actually "died" on the table, there would have been a code and staff would have rushed the room. Instead it was presented like no big deal, an afterthought.
shareI think whatever was in Vic is in her now or he thinks so...
shareI got that same impression too! I had to go back & replay this several times but it appears that she opens her eyes & they're dark (maybe it's just my screwy vision b/c it's bad, really bad! I'm actually losing & have lost a great deal of sight & won't have what little I'm left now forever b/c of a retinal disease, optic nerve defect, & damage from 2 of 4 autoimmune diseases I have too) but it really seemed like she had dark eyes when she opened them some & then closed them again - which is what I believe sent him outside to either check his phone or make a call & found he had the message with the info he asked about earlier.
It could be simply have been a code; I've had family members who were/are old who have coded along with some that aren't so old but they were able to bring them back; hwr, in this series with what James has experienced from the start, I believe it gave him pause to believe it was something else, esp after he checks his phone!
Glad I'm not alone in this line of thought. :)

I don't think he made a choice. I think he went to the hospital because it was the right thing to do.
shareI agree with jenkosko here. Being present for the birth of their child is the least a father can do (unless something really serious prevented him). It wasn't necessarily a choice, even though when Kate says she understands and lets him go, it does seem like she is accepting the fact he should be with Sara, so...
shareI don't think he's chosen yet.
Not a fan of the whole "love triangle" idea though.
