The last scene with him in it at the end of episode 8 shows him and Payne together outside the Pawn shop after Swagger intercepted Payne there and took the USB drive and the diamonds.
Then at the beginning of Episode 9 BEFORE the credits roll, they show Meachum wrapped up in a bloody plastic tarp and being transferred from one van to another by the Russians.
He wasn't dead when they threw him into the van, but since he's being carted off for disposal, it would have been more merciful if he was. He's either being buried, shredded, or burned.

Yes, Tom Sizemore was fired for reckless behavior on set, rumored to be caused because he was drunk or on drugs at the time. Perhaps they planned to show in more detail what happened to Meacham, but I am quite sure that his fate was part of the plot from the beginning.
It's too bad Sizemore is so self destructive because he certainly was very good in the show, just the right balance of cocky and manipulative.

Sizemore backed a car over a stuntman. Fortunately the stuntman was not seriously hurt, but no one knew that when they called 911, particularly the stuntman. Anyway, he's suing the production and Sizemore was fired.

Years ago Sizemore had to do a urine test to stay out of jail and got busted. He was wearing a prosthesis that held older and clean urine. The urine was also tested for temperature and since it wasn't "hot" he obviously got busted!!
The guy was always a good actor going way back to Saving Private Ryan and other movies, but his demons always, always got to him. Much like Charlie Sheen, without the dragon DNA.
