Is this supposed to be a family comedy or workplace comedy?

just saying-- there's kind of an imbalance here- it would probably benefit the show more if the producers concentrated on one format or the other. Either focus mainly on Ken's home life or focus on the wacky hospital work life and co-workers.

It seems like the producers didn't want to choose, so they went with both.

I like it as both. Some episodes one of the places is better than the other

A lot of sitcoms focus on both. Frasier immediately comes to mind. Other sitcoms like The Andy Griffith Show, Scrubs, The Middle, Home Improvement, etc. effortlessly blended both. I don't have a problem with it. I personally enjoy the show more when it focuses on the clinic.
