Seems like Dr.Issacs is addressing Umbrella board and a person in a wheel chair at the opposite end of him. Could be old Alice.

I think that Umbrella Board scene is a flashback set not long after Raccoon City incident. Let's say between Raccoon City and Tokyo outbreaks.
