Early in the movie, it's kinda clear what's going on. The main protagonist (Sam) just couldn't get a clue. (Duh). Movie was okay.

Are we to understand he's going to come out of the coma in the end since he made contact, or that him going onto the roof means he's letting go and dying?

The doctors name was Lillith.. That was the main clue that this was a battle for his soul as Lillith is literally the mother of all demons.. Maybe he just wasn't a religious man and didn't know that.
