Who was it at the door when...

...David was talking secretly and then explained it was people on foot looking for another party? Was it really other unsuspecting victims invited to a different suicide party that night?

I assumed it was other members of the cult, perhaps giving him his lantern or making sure they were still on board? When the teacher girl leaves I thought we would find out for sure - anther loose thread though.

The commentary track on the dvd with the writers and director reveals that Pruitt did indeed kill the woman who left early. There's a shot later in the film with Will standing on the balcony, but we only see part of it because as the camera pulls back it shows her in the bushes below the house dying. They cut it because it revealed the threat too soon and destroyed the ambiguity of the situation.

"I was cured all right!"

Yes, this is a bit odd and is not explained. I thought maybe it was the police who had found a dead Choi and were looking for clues. The only explanation would be that it was other cult members touching base or something; but what a bummer to have to organize your mass killing by going door-to-door, as phones don't work there.
