Now, what if the Hulk picked up Thor while he was holding Mjolnir. Wouldn't this mean that Hulk picked up Thor's hammer?

if the only time he can ever pick it up is while holding thor then clearly he's not the worthy one
Since Vision is kind of an advanced android, him picking up Mjolnir doesn't count.
That was the joke at the end:
Cap: But if you put the hammer in an elevator...
Stark: It would still go up.
Cap: Elevator's not worthy.
That's funny. I'll keep a lookout for that scene the next time I watch the movie (I haven't seen it since it debuted in theaters).
