Towards the end when Phoebe tells Adam that it is not fair she still hates everybody but him. Does that mean she wants him back or not ??

I took it as meaning that while a part of her definitely wants to have a relationship with him, she knows that he's not stable enough for her - at least not at this point in his recovery. So, no, as much as she likes him (which accounts for her saying that it's 'not fair' that she still 'hates everyone but him,' she knows it's BEST not to take him back.
