The ourboro (snake thing Rose read about from the book)?

I don't know and I don't think the reason for the happenings in this movie was ever specified.

Since Rose was apparently high and nobody else saw it, my guess is: nothing. She wasn't actually scratched, it was just her way of experiencing the presence of the time loop aka Ouroboros. Just as she saw the other people as corpses "before" they died.

Still doesn't explain why only Rose saw the scratches.
