Why is the main character almost always shown lying down? Couldn't his cot tilt, or show him in a wheelchair? I think I would have liked him better if I wasn't distracted by always looking at him sideways.

Well, considering the film is based on true characters, I have to assume that Mark couldn't sit upright. Some people who are paralyzed pass out when they're put in a seated position because their blood pressure drops.
Imagine how hard it was for Mark to see the world sideways.
