How did player end up in Comstock universe in the first place

At the very beginning of the game the player is already in Comstock universe; how was he brought there into the situation where 2 selves exist in same world? If it was Elizabeth, when does the story say she did that?

The Leutces opened a tear to his dimension. Remember the ending where Elizabeth says Booker sat alone in that room for 20 years with his guilt until a man showed him a way they could be together. The man was Robert Leutce.

That's right and it even has a scene where there is a tear in his room with Lutece standing there coaxing Booker out.

QUESTION:
Did Booker change his name? Or was Comstock his original name? Or does the whole dimensional thing mean you simply "end up" with a different name than you originally had? In other words why 2 different names?

Also, did anyone else get that A.D. on Booker's hand is not only a Christian separation of time, but it's his baby's initials. Anna DeWitt.

I like wearing camoflage; it helps me hide... coffee stains.

After the baptism Booker re-invented himself as Comstock.

Anna Dewitt was what the brand meant.

The game shows you exactly how it happens. Robert Lutece opens a tear and coaxes Booker into coming through. The shock of being in a new reality and it altering his memories, causing him to collapse. So The Lutece twins drag him to the boat and head off start rowing towards the lighthouse. And thats where the game starts, with Booker having invented new memories to compensate for the differences in the Comstock reality.
