Sorry, I didn't quite get your question? Were you asking about whether Rosie/Alex end up together… and if so, do they have any children. The book pretty much follows the life of Alex and Rosie from when they became friends at a young age (age 7) and follows their lives as they get older (it ends when they are about 50-ish years old). As the trailers suggest, both are best friends but due to a range of different circumstances, their relationship never develops any further - it is very much a will they/won't they style novel.
Here are the answers to your questions: the circumstances that prevent Alex and Rosie from developing a relationship include Rosie having a baby (i.e. the result of her one-night stand with Brad), marriages, divorces, dating other people, etc. It is only at the very end of the novel - on the last few pages - that Alex and Rosie finally get the chance to reciprocate their romantic feelings for each other. By this stage, both characters are well into their fifties (and it is implied that Rosie has gone through menopause) and have had children to different partners… although both are already divorced. It's somewhat of a bittersweet ending as they both finally come together, but it took a long time.
As side note, I'm not sure if this is the plot/time frame they will be using for the film. It seems like Lily Collins/Sam Claflin will be featured heavily so they might not extend the story all the way up until the protagonists are 50.
