I thought it almost relied upon the viewer having read the book. I remember such long passages of dialogue that was just the tapeworms. In the film the tapeworm was replaced by a doctor wasn't he? God knows.
sharei havent read the book, and it didnt confuse me, in fact i dont know what would be confusing about this movie, despite all its awesomeness and weirdness, its pretty straightforward in its structure.
