What are the theories on his "death" and return?

In State of the Union it's mentioned that he died in Bora Bora but in the short lead-in to that movie he's apparently killed when his loft in Los Angeles explodes courtesy of the bad guys in SotU and a piece of scalp bearing his distinct tattoo is thrown from the explosion. So, are we to ignore the short film of his "death" & infer that said non-death was in Bora Bora, or take it as canon, assume that the NSA was mistaken or mislead about the circumstances & location of his "death", and that it was an elaborate ruse complete with a body double (much like how Diesel's body double was used in his place)?

If it's the latter then why go to all that trouble? Did Xander or some part of the NSA know what the SotU bad guys were planning? If it's the former and his non-death was in Bora Bora, might he have staged it as something accidental that the NSA wouldn't follow up on, like a surfing or hang gliding accident, to get from under their thumb?
