It makes me wonder, though, why do they show one disclaimer for some of the episodes saying the deaths portrayed are real and another disclaimer for the other episodes saying the stories portrayed are based on real deaths? Is the former meant to signify that the deaths happened as portrayed, or is there really no discrepancy between the two?

I think the OP did not know that there was a real show called 1000 ways to lie. They are trying to smart off by saying lie instead of die because they feel the deaths are fake. Of course since the show says "Based" on true events i dont consider it lying. I will say they embellish a little(some more so then others) but the deaths are still real.
