This is what I guessed, but he was much stronger than her. So, it does not much add to the logic of the story.

Maybe she sneaked up on him when he had his back turned. The fact that we don't know makes it scarier than if we saw it IMO

Good point. You have a sense for subtile horror, in these days a very rare quality, where all is about gore.
Nevertheless it's not that logical. The old woman was dangerous because she has bitten the old cop before. The same happens later with the small girl. The second cop is acting like she hadn't bitten her mother before. Stupid.
I think the movie would have been better if those things had been executed more accurate.

Any ideas?

The old woman bites him. He tries to go to the stairway and yell for help. Gets to the edge and can't speak. Ends up passing out and falling over the railing.

He fell because he was attacked by the old woman.



Also according to movie trivia section:

"During the filming of the scene where the young fireman falls from the stairs, not a single actor knew that was going to happen, so the reactions we see on the films were the real reactions of the actors themselves. "

I imagined he was struggling with her and the result was she pushed him out the apartment, into the corridor and then when bitten he fell backwards into the stair well.

A woman can be any shape she wants.
What about a hexagon?

Because the "disease" was really demonic possession (I liked that twist) a demon is much stronger the a mortal.

Drunk people are much stronger than usual because they don't know their strength... I assume the same goes for a possessed old lady.
