Maybe she sneaked up on him when he had his back turned. The fact that we don't know makes it scarier than if we saw it IMO

Good point. You have a sense for subtile horror, in these days a very rare quality, where all is about gore.
Nevertheless it's not that logical. The old woman was dangerous because she has bitten the old cop before. The same happens later with the small girl. The second cop is acting like she hadn't bitten her mother before. Stupid.
I think the movie would have been better if those things had been executed more accurate.

Also according to movie trivia section:
