The element is MDMA. Nothing "supernatural" about it though. It' just chemistry.

I would have said it was purely about mental illness thought a lot of the imagery and sound effects stem from the horror genre. Also, reading the Trivia on IMDB I found - Although the film was never marketed as such, Darren Aronofsky has always maintained that this film was meant to be a psychological horror film.
