In the hospital? was this for real or an illusion? Anyone know?

Nina did it because she wanted to ruin her, so she can to be "perfect" ballerina.

There's something wrong with Esther.

Do we know that for sure though? We also saw her kill Lily but that turned out to be an illusion, so the same might go for what happened to Beth.

They looked fake; it was too easy for the nail file to penetrate the face. So I assume it was Nina's hallucination.

The fact that Nina appears at the hospital very suddenly also contributes to the hallucination theory. And why would a nail file be so important to Beth? We don't even see Nina take it from her, just the lipstick, and then we see her leave without taking anything else.
