And yes, she absolutely looks way older than 16. But the fact that she does, makes Hank's character so much more sympathetic. IMO If her she looked anything like 16, the audience would tend view Hank as unforgivably depraved, rather than as the forgivably depraved and lovable character that he is.
She did look older than her age and that's why Hank had sex with her. He had no idea at that time how young she was. Once he found out he never crossed that line again. Lew Ashby knew how young she was and still was with her sexually

No, it wasn't legal since she was 17 when Lew had sex with her. Karen clearly states this in S2E8, when she's having an argument with him about it. He says he thought she was 19.
