Why was he surprised to see..(spoiler)

Judy in the house at the end? He knew all along he was was having an affair with her (we the audience weren't supposed to know.) His reaction made no sense.

Actually it did make sense because he didn't know where the house was where he was meeting Judy. He was supposed to call Judy to get directions after he dropped Abby off. That's why Judy asked him how he found the house.

When Tom directed him to this house, he thought it was just someone that Tom wanted him to kill. He had no way of knowing that it was Judy in the house until he saw her.

close but not quite. Like you correctly said Neil hadn't been there before. 'There' being Karl, his boss' lodge. Therefore when Tom tells Neil they are going to Karl's after all he has no way of knowing who the lodge belongs to and has no reason or will to doubt Tom by this point.
