Im fairly sure that he punched the kid, to send Reaper a message. A lot of the violence (except what happened to himself) was not shown in the movie most likely due to it being like a recollection.
Also we are talking about the slums of New york, probably just bad parenting or parents just not caring. They were probably drug dealers themselves and stuff.
I wasn't positive at first. But I bought the DVD and in the deleted scenes feature, there is a scene of all the kids attending a Greek feast. The little boy comes in with his mother, and he has a thing over most of his face (bandages, I guess). The mother goes over to Antonio (first she goes over to Dito, then I guess realizes Antonio is the one) and starts yelling at him and pushing him.
