Not the same actress. Also in the story, I think Nurse Evelyn represents all women more than a specific woman. First Roy is so depressed by the woman he lost, then as he feels the tiniest bit better, the symbol of all women turns out to be hitting on the sly with someone else too. That's when Roy really loses it.

We only see what Alexandria imagines from Roy's words, so that's who she chose and then went with it especially after Roy needed to let that character go too. That's my perspective, anyway. :)
