The fear and excitement that floods your body with adrenaline can have some really weird effects, especially if you haven't experienced it before .. it can seem dreamlike and you can have trouble with motor control..

If you read first person accounts of shoot outs back then (and even more recently), it was (is) not usual for both shooters to frequently miss at close range, even several shots. They're in a hurry to get their shots off and "slapping" the trigger (a right hander will tend to miss right, left hander left), and small motor skills (such as used in aiming a pistol) are one of the first things to suffer during an "adrenalin dump". A common observation made by (in)famous gunfighters back then was that when it came to a gunfight, slow and accurate always beat fast and wild. In this case, they were both fast and wild, but Bob Ford's coup de grace wasn't.
