She had the voice even back when she was in college. When the heck did she start smoking? I often wonder if I could do it with a woman who looked like Dr. Girlfriend who had the same voice. Would that kill the mood?

Exactly, at this point her voice is just part of her. It'd be weirder to hear any other voice for her at this point. Honestly unless I actually stop and think about it I always forget she's voiced by a dude.

I am still not sure. I can't prove this but i think you see her with an Adams apple in season one. It is not there later

At the end of Season Two's episode "Hate Floats" Phantom Limb said something to the effect that The Monarch was only guilty of not being able to let go of Dr. Girlfriend due to the fact that "no man can resist her charms". Brock replied, "No man? I'm still looking for scars on her!" He also queried Phantom Limb about it earlier in the episode.
I seem to remember reading that the show's creators were using the transgender thing as a sort of "red herring" for the cliffhanger ending of Season Two when Dr. Girlfriend (now Dr. Mrs. the Monarch) informs The Monarch that "there's something she has to tell him." Of course, as we see in the opening episode of Season Three, her gender has absolutely nothing to do with it and we later learn in the episode "Home Is Where the Hate Is" that her voice sounds like that due to her heavy smoking habit.
Apparently that was the only reason he turned down the show's hottest character... I thought it was bizarre, too, since Brock certainly hooked up with some pretty skeevy women, particularly in Season One; Lt. Anna Baldavitch in "Careers in Science" comes to mind, as well as the hooker in "The Terrible Secret of Turtle Bay"!
What I find ironic that the woman he is with now is far more masculine than Dr. Girlfriend ever could be, even with the voice.

Yeah, I found that pretty bizarre as well. Apparently a lot of the fans weren't too thrilled with it, either. Poor Brock; he slept with almost every woman on the show except the two hottest ones (Dr. Mrs. the Monarch nee Dr. Girlfriend and Molotov Cocktease.)
