Sleeping with a patient-Billy.
Quickly setting up the idea that Colin is the father with that sonogram picture.
Say, did we actually see her and Colin having sex? Don't remember the scene?

They either did sleep at least one time together or poor Colin had no idea where babies come from.

Madolyn is practically thrusting a banana into the camera during that scene. It's about as subtle as the rat in at the end of the movie.
