Are we sure that the baby is Sullivan's?

I would say we are NOT sure. I'm sure people have their theories and some hope it wasn't Sullivan's but I don't think there is a way to tell from the movie.

I think the implication is that the baby is NOT Sullivan's. As said before, we can't be sure, but the events in the film portray Sullivan as having episodes of impotence. Then, when she told him she was pregnant, he was not only surprised, he was a bit confused.

Couldn't agree more. I guess it's not outright said, so it could be SulIivan, but personally I don't think so. I just rewatched this movie and I always felt it was heavily implied that the child was Costigan's. I always interpreted it that Madolyn knew herself too.
Not only was Sullivan portrayed as impotent and really surprised to find out she's pregnant, but shortly after when she discovers that tape while he's in the shower, she basically tells him as much. She locks herself in the bathroom and shouts 'And I thought I was the liar' (It could simply imply that she had to cover up her affair, but it was said directly after the scene in which she tells him about her pregnancy) We see her later struggling to find a good way to say something 'important' when Costigan gives her that envelope. Costigan tells her to save whatever she has to say for later and tell him in two weeks , but of course he dies shortly after. At the funeral, she completely ignores Sullivan when he asks her 'What about the baby?'. Of course, she could simply want to raise the child on her own cause she hates his guts, but I see it as her kinda going: yeah, the child wasn't even yours so *beep* off.

I've always been 98% sure that it's Costigan's.

I think you're right - I think the father is Billy Costigan. Up till now I thought it was Sullivan's but now it looks to me like it's Costigan's.

I thought she wanted to tell Costigan, but thought different? I always knew it was Costigans, just because I think Sullivan was impotent.

No way, he and the shrink haven't slept together in a while now, it is clearly Billy's kid.
