i think Nanny was always pretty. She is shown as ugly because that is how the kids and the dad see her. The kids because they do not want to like her, the dad because he doesn't think she can handle them. When she gains their respect/affection they see her a different way and at the end she is pretty.

They say that theory in the extras on the dvd. There's also another theory that the deformities represent the kids' bad behavior and each time they get better the deformities disappear.
