Major plot hole (spoilers)

Maybe someone can explain to me, otherwise i can only see it as a huge plot hole... maybe i just got the end of the film completely wrong. But if all 3 of them died in that car crash, simon as well, then what is this whole story with Anna? I assume the car crash took place in 2000... like it was said, and then what happened? we dont actually learn anything about the time between that first car crash and his 2nd accident in 2002... all we know is that anna blackmailed him and married him. Then of course theres the whole fact that in 2002 he was seeing Kate, i think her name is... who also died in the crash... so what the hell was going on? was that just his imagination. I DONT KNOW.

WHY if he knew he was going to push (accidently) his brother off that balcony, why did he go up there and follow him? wouldnt it have been easier to have just let his brother be all pissed with him, then trying to explain things, when you know it will just lead to that accident.

To many things that make no sense.

Has anyone figured out the chemicals in his bloodstream thing? Was he poisoned? I never heard anything about that after the first couple of times it was brought up. I never heard anything about it at all during the last half of the film. Am I missing something?

There was no 2002 for him, it was all his imagination. He died in a car accident as well as his bro and lover. He made Anna in his dream right b4 death.

Well, I think differently...
In my interpretation of the movie what happened was that the only person who died in the 1st accident was Claire, that's why one of the first scenes of the movie is the final scene where Simon is arriving to his brother's home and talking with him about what happened.
So, through all the movie the point is to discover what really happened while in his mind, Simon recreates a different story using all the stimuli around him (like Anna and Travis(?) who are the paramedics that rescue him in the car accident).
I also think that the movie push a little bit the concepts of time traveling and parallel realities. I think we can say that Simon was in a live/dead experience and he was jumping into different realities.
In the end, Simon realizes that Claire was the only one who died in the accident and that he can't really change anything about that, but he can choose the reality where he is living, so he returns to the point where everything could have changed...he dies and he goes to a parallel reality or a different state of mind (and in there is father is the doctor, Claire is still alive, his brother has died...)
