They never said what happened to her. Later in season 3, Ned listed her among the dead Mallrats.
When asked about character's deaths in an interview, Raymond Thompson (the show's creator) said that as far as he was concerned, Patsy (and Danni) were both still alive. I guess he used the rule "If you don't see a body, they might still be alive." We saw Dal's body, so we know that he's dead. We never saw Patsy's body, so she might still be alive.
As for why she left, a number of other cast members had to leave, either permanently or temporarily, due to a series of important tests that they have to take in school at a certain age in New Zealand. I've always assumed that Patsy left for the same reason. Cloe left at about the same time.

The cast member Sarah Major was unable to continue as Patsy because her agency and Cloud 9 had started butting heads so they pulled her from the show just like that. Sarah personally wanted to continue on The Tribe but her agency wouldn't let her. Who knows if she would have ended back up in the show should the dispute have reconciled and The Tribe carried on.
Cloe (Jaime Kaire- Gataulu) was a different reason. She and the person who played Ved (Dan Weekes-Hannah) blew off a major day of filming and cost the studio lots of money. That is why Laura Wilson (May) returned in her spot and Ved was ultimately replaced by Mega (Calen Maiva-Paris).

Thanks for the replies, Ned did list her as dead but there were many other references (its fresh in my memory)
Amber calling her "poor Patsy"
Luke "I couldn't get to patsy in time"
Ellie telling Jack about Cloe and Ryan taken away and then says "Patsy she..."
To me that means she died and I think thats what the Cloud 9 wanted otherwise why not list her as working in the mines with Cloe and Ryan but like you say if you don't see a body there is always a way back.
I wish May never came back though. Awful character!!
Season 3 certainly was the turning point.

I was talking about the first time she left, not the second. Early in season 3, Cloe was sent off to the mines by the Chosen. She came back later on in the season after the Chosen were beaten.
Thanks for the info on Sarah. I always wondered why she disappeared so suddenly.
