I think the main reason O-Ren was so offended was because he insulted her parents.

I think it's more because being an ethnic minority in Japan, especially among the notoriously conservative yakuza, is a precarious position. She had to have respect and so as soon as he expressed his bigotry and his unwillingness to accept her as a leader on the basis of her race, she nipped that in the bud right quick. In order to stay on top, she has to have a no-tolerance policy on that.

I'm not sure it was personal.... not solely anyway. She was vying for leadership among the Yakuza families and one boss not only disagreed with it but also disrespected her in front of the other bosses/lieutenants.
