He hadn't physically been to Rome and thus it would be hard for him to be recognized. When he met Lucius, who had said that he watched the games with his uncle in that very conversation, it meant that either Lucilla (who he didn't think he could trust at that time) or Commodus himself, both of whom actually did know him directly, could have been in the immediate area and spotted him. Especially if they were keeping a close eye on Lucius. Or at least that's how I see it.

The way I see it at least Rome's a big place bustling with multitudes of people, making their chances of intersecting before the coliseum (which is why he picks a helmet that partially obstructs his face before entering), especially given their positions as emperor and slave/gladiator,unlikely. I also don't think Maximus would have much of a reason to think Commodus would be out amongst "the rabble" himself. Just taking more caution when he unexpectedly meets Lucius, as it would up the chances of either he or his mother being around.
shareThat makes sense: he wasn't surprised that Commodus etc were in Rome, he was nervous about being in such close proximity to Lucius since it meant he may be recognized right there, which was too soon. I think he played that scene a little too nervously though.
