Old woman - patient - smoke got blown into her face

Why was there a very old woman as a patient in the ward with the girls/young women?

She just really stood out, and I'd guess they had other wards for older patients.

Any idea why she'd be there?

there is no segregation in a mental institution, but that being said there could be depending on a variety of things. in this place and time there was none

There may not have been segregation THEN, but there is now. It's done to protect children from adult predators. There are also two types of juvenile wards depending upon the hospital. One for teens, 13 and up, and one for 13 and under. I know someone who was placed for a time in the 13 and under ward a long time ago. And it's unreal just how young they'll admit kids now. Apparently there was a couple of kids as young as FIVE in the ward.

She wasn't a patient. She was a nurse babysitting them.

Is she the same woman who plays Mac's mum in It's Always Sunny?

Anyway, I imagine they have more than one female ward, and probably mix up the people that are in there to an extent. That said, it's the 60s, so just wherever they could put her. They didn't really care back then.
