His wound...

One thing I wish Shyamalan would've done is pepper in tiny subtle references to Malcolm's gunshot wound. For example during the "Next Fall" scene on the bench, if Malcolm would have winced just a bit and maybe gently grabbed his side for a moment, that would almost completely solidify the (ultimately false) notion that Malcolm has nearly fully recovered from his wound and has started to resume his work. He does this once already when Malcolm breaks the door at Ana's shop.... he hurries away gripping his side as if he is in physical pain. I think just maybe even one or two small instances more may have kept the sharp people from guessing the end.

He was dead, hence no pain.

Most people/audience didn't know Malcolm had died, so there shouldn't have been further examples needed for the audience to know that he's "alive".

After the intro sequence, is the FURTHEST thing the filmmaker wants the audience to think about (Malcolm shot, lay dying) -- but to focus on Cole and his issues, not Malcolm because of implications.
