There are several possible sources to the term, one of the most popular suggestions seems to be:

in the Victorian era it was supposedly called a "below job" as the girl would go below the table and use the table cloth to hide any activity. this allowed for cheaper transactions since no room or bed needed to be rented. slang eventually developed from mispronouncing below as b'low

no one bothers to correct the name since a "below job" has little meaning today in a world where prostitution is mostly illegal

add to the fact that the man in the situation does eventually "blow" a load; and the nuance of the term blow being an ironic counterpoint to most of the activity (suck) involved and pretty much no one cares enough to try and change the entire worlds view on what it should be called officially
