1. Technically, yes, her body would still be injured. I have a similar problem with the restoration of the Beast. He gets stabbed in the side by Gaston, but somehow, his magical transformation heals him even though it was just to transform him from beast to man.
2. I'm pretty sure Meg knew she was dying in her final moments, and from what minimal details I can gather from the film, a person's soul is alive and conscious when they are traveling in the River of Death. Remember, at the beginning, two souls reach out towards Hades when he arrives back at the Underworld, but Hades whacks them away. The rest is left to imagination when it comes to Meg. I imagine Meg's soul came into the Underworld alive and conscious of where it was, but slipped into unconsciousness after a short while.
This movie has a lot of problems, and most of them are story-related. Being an animated film is not really a valid excuse anymore since other animated films don't have as many contrived plot points.
