Does she fake an abortion or miscarriage?

I've always wondered this. When Alex sees Isabel in the hospital, the things she tells him sounds like she wants him to think she aborted the baby because they could have never gotten married if there was no baby so she killed her, but that's a really heavy lie.
Opinions?

She implied she had a miscarriage. She went to the hospital because she was in pain. Nothing was wrong but she led him to believe she had a miscarriage because she wanted to leave him before he left her.

I think it would have to be a miscarriage. With her strong Catholic faith I don't think he would have believed her if she said she had an abortion.
