Did Arnold have some kind of skull deformity, or is that shape of head considered normal in his cartoon world? Neither of his parents had his shape of head, nor did any other character.
Also, why did one of the other characters appear to have a phallus growing from his forehead? It was too high up his face to be a nose.

Nothing's "wrong" about his head, he's a cartoon character in the style of his creator. That's he's identified as "football head" by other characters is simply because his head does resemble one, but it's not a physical issue.
